The Health Home program coordinates care and services for Medicaid beneficiaries who have chronic health conditions. The program addresses the whole person, not just their symptoms. Health Home staff does not come with pre-set agendas or expectations but instead works with clients to develop an individualized holistic care plan that is guided by the client’s personal goals.
There is no cost for individuals who participate in the Health Home program. Participation does not change the client’s benefits, or any current healthcare or service providers.
Full Life Care is a not-for-profit health and social services agency, and is the only community-based Lead Health Home Network in King County, and is a care coordination organization. Full Life’s Health Home network also provides services in Pierce and Snohomish counties. Health Home originates from the federal Affordable Care Act to support individuals, reduce the cost of healthcare and improve health outcomes.

Health Home care coordination can take many forms based on the client’s goals.
Examples of services include:
- Assistance with medical provider referrals.
- Locating and maintaining safe and affordable housing.
- Identification of services available in your community (such as transportation, food assistance, and financial counseling).
- Assistance with hospital discharges.
- Connecting to in-home care services.
- Assistance connecting with other long-term care services.
- Locating social activities and organizing outings.
- Accompaniment to doctor appointments.
