Adult Day Health: Get out of the house and exercise

A fun place to go for a few hours a day to get some exercise, check-in with the nurse, to socialize with others and more. We focus on supporting the independence of older adults and people with chronic illnesses, memory loss, and disabilities who wish to continue living in their community.

Home Care: Helping you remain at home

In-home caregivers provide care in your home, helping with personal care, cooking, laundry, housekeeping, shopping and more. Caregivers help clients remain living independently. For elders and adults with disabilities who live alone or with family members.

Loneliness: Connecting elders with volunteer companions

ElderFriends volunteer companionship program connects elders experiencing loneliness with trained volunteers for regular visits and occasional social gatherings.

Care Coordination: Helping Medicaid beneficiaries improve health outcomes

The Health Home program helps Medicaid beneficiaries get the care and services they need. Full Life’s care coordinators meet clients monthly to offer support, including help finding healthcare providers, social activities and more. You must meet certain criteria. If eligible, there is no cost to you, and there is no change to Medicaid benefits.

Housing Support: Aide to find housing or stabilize current living situations

Help finding independent housing, or with issues that put current housing situations in jeopardy. Full Life Care helps individuals find and maintain housing, both short-term intensive support and continuing long-term support for a variety of life’s needs.
