It’s all part of living: At some point in our lives, many of us will experience a chronic illness, advanced memory loss or disability, or care for someone who does. Others may face mental health concerns, housing instability or are living in isolation.
Each of us at Full Life Care is dedicated to enhancing the quality of life for the people we serve. We are committed, through our health and social services programs, to enhance the independence and well-being of all clients and program participants, and to provide respite for caregivers.
We provide a range of programs to help clients reach goals and achieve small miracles every day.

What do you need?
Day Health Programs: Get out of the house and exercise
A fun place to go for the day to get some exercise, have a nurse help with medications, socialize and more. For elders, individuals with physical or developmental disabilities, Alzheimer’s, dementia, traumatic brain injury and chronic health issues.
In-Home Care: Someone to help at home
Help with personal care, cooking, laundry, housekeeping, shopping and more. Our in-home caregivers help individuals remain living independently. Home Care aides help with bathing, personal care, and medication reminders, as well as being a companion on appointments and on errands. For elders and adults with disabilities who live alone or with family members.
Counseling: Someone to help work through challenges
Help with mental or behavioral health challenges. Our masters-level counselors help with a range of challenges using evidence-based methods. We specialize in geriatric mental health, developmental disabilities and brain injury.
Help: Identifying needs, navigating the care system
Help to identify care needs, layout a plan and navigate the long-term care system. An ideal service for individuals who have limited or no support locally. Our care management team can serve as a liaison between you or your loved one, and with family members, care providers and others. (This is a private-pay, fee-for-service option).
Care Management and Consultation Services
Loneliness: Socialize and have regular visits from a friend
Opportunities for older adults to socialize, have a friend visit, or maybe go on an outing together. This volunteer companionship program connects an elder experiencing loneliness or isolation with a trained volunteer. You both commit to regular visits.
Care Coordination: On Medicaid and have a chronic health condition
Many individuals want or need support to set and accomplish life goals. May include help to find healthcare providers, social activities or learn to read, for example. Eligibility criteria applies. If eligible, there is no cost to the participant with no change to Medicaid benefits.
Housing: Find housing or stabilize your current living situation
We have several programs to help find independent housing and to address issues that put current housing situations in jeopardy. Contact us for information or for an assessment.
