Full Life Care’s Foundational Community Supports housing case management team works to help eligible clients throughout King County find and maintain independent housing. We work with adults who have Medicaid insurance and complex care needs.
Our team’s housing case managers work with clients as long as support is needed, even after a lease is signed. Together, housing case managers and clients develop a housing goal plan based on individualized and self-identified goals. There is no cost to the client.
Ongoing supports include:
- Assessment of Housing Needs
- Identification of Housing Resources
- Connection to Community Resources
- Housing Application Support including Transportation
- Support in Obtaining a Lease
- Training in Life Skills
- Crisis Management and Tenant Advocacy

Who is Eligible
To qualify for the Foundational Community Supports supportive housing program, you must have Medicaid insurance, meet at least one needs-based factor and at least one risk factor.
Needs-Based Factors
Must meet at least one of the following:
- Currently homeless and have a disability or complex health need.
- Need help with activities of daily living, which include activities such as mobility and personal care.
- Diagnosed with a behavioral health need or substance use disorder.
Risk Factors
Must meet at least one of the following:
- Diagnosed with more than one chronic health condition.
- Have a disabling condition and have been homeless for at least one year.
- Lengthy or frequent stays in hospitals or skilled nursing facilities.
- Lengthy or frequent stays in adult residential care facilities, such as adult family homes or boarding homes.
- Experienced high turnover of in-home caregivers.
Next Steps
If you believe you qualify and would like help finding housing, contact us and we’ll complete an assessment. There is no cost for you to apply for the program or to receive services if you are eligible.
When approved, we will provide a timeline for the assignment of a housing case manager. Your housing case manager will meet with you regularly to develop a housing goal plan and identify steps to reach those goals. Once you are in housing, the support does not end.
Our housing case managers continue to work with you as long and as often as you like on new housing-related goals.

About the Program
Foundational Community Supports is a program funded by the Washington State Health Care Authority and overseen by Amerigroup, a statewide third-party administrator. Amerigroup contracts with agency providers, including Full Life Care, to provide services.
Full Life Care provides supportive housing services. Supported employment services also are available through the Foundational Community Supports program; however, Full Life Care does not provide supported employment services at this time.
