Health Home is a service in addition to your Medicaid coverage. It is a voluntary program that offers you further benefits at no cost to you.
Health Home services help you coordinate your medical and social services support. The goal is to support you to improve your overall health and well-being.

Health Home beneficiaries are enrolled in the Health Home program by the Health Care Authority. Because you have Medicaid insurance and meet health-related eligibility criteria, you were enrolled in this program and can begin receiving services right away.
Full Life Care is a not-for-profit health and social services agency that manages your Health Home network.
You may be eligible if you are:
- On Medicaid; and
- Have one or more chronic health conditions.
What will Health Home do for me?
Health Home provides you with a skilled care coordination professional who will meet with you monthly. The care coordinator will support you in creating a Health Action Plan tailored to your goals.
Health Home care coordinators can:
- Meet with you monthly at your home or another location of your choice.
- Make appointments for you with your primary care doctor or specialists to help you get the care you need.
- Provide discharge planning assistance if you go into the hospital.
- Help you understand and better manage your health conditions and related symptoms such as asthma, diabetes or high blood pressure.
- Access resources to help you achieve your goals.
- Assist you with housing applications, connecting with legal resources, transportation providers or other needs.
There is no fee for Health Home care coordination services. You can receive these services for as long as you’d like.
Will this affect my Medicaid coverage or other health and social services?
No. Your current Medicaid benefits will not change. You may also keep your current providers.
Your Health Home care coordinator may refer you to additional services.
How do I sign up?
A care coordinator will contact you. You are also welcome to contact us to schedule an appointment right away by calling 206.231.0207.
What if English is not my preferred language?
We have telephonic and in-person interpreter services available and at no cost to you. Some of our care coordinators are also bilingual or multilingual in Korean, Vietnamese, Russian and Ukrainian languages.
What if I don’t want Health Home services?
Health Home is not mandatory. Please contact us if you’d like to opt out of the Health Home program by calling 206.231.0207. You will need to provide your full name and birthdate. If you opt out of the program, you will be unenrolled by the Health Care Authority.
If you change your mind, please call us at 206.231.0207 and we will help you determine if you can be re-enrolled to begin services.
