Full Life Care – The Power of Connection

Full Life Care Service Coordinators provide critically important outreach services to our clients who reside in Public Housing environments. Many of these individuals struggle to maintain basic necessities, while struggling with numerous disabilities or physical illnesses.  Much of the work Full Life Care provides to public housing residents is intended to improve their access to healthcare, and to improve outcomes around basic social determinants of health.

This spring during an outreach visit with a public housing resident, Service Coordinator Michelle Lewis encountered a single mother struggling with financial challenges and compromised health. The resident shared with Michelle that she is living with stage 4 cancer. She learned that in addition to completing chemotherapy treatments on her own at home, the resident was attending monthly chemotherapy sessions at a local cancer center. 

Michelle asked the resident if she would like company at an upcoming appointment. The resident was moved to tears by this offer and confided that nobody had ever offered this type of support to her.  Michelle made a plan to accompany this resident to an upcoming chemotherapy treatment appointment later that month. “Sitting beside her during treatment reminded me that connection is often found in the simplest acts—just showing up, listening, and being present. It was an honor to support her through such a vulnerable moment. Experiences like this are why I love what I do. Every conversation, every visit, is a reminder of how deeply human our work is.” — Michelle Lewis
